We only share required data with your provider to verify your diagnosis. Any other data that is shared with other services are done so with your permission for communication reasons. We may also use this data for applications for grants or for research reasons from verified institutions after the data has been anonymized.
How long we retain your data
This website retains your data until we enter it into our own local databases. That data is held indefinitely as a hard copy in a secured location. You may request to view this file at any time as a member. You may also request that the file be destroyed if you no longer wish to retain your membership. We may keep certain data such as your full name or other identifiable information for our own records for research and funding reasons.

Where your data is sent
All data sent via the application form is sent to our membership team on submission via email. Any Protected Health Information (PHI) that is transmitted by your provider at your request is sent to us via fax or email. This data is printed off and placed into your physical file and deleted.
